Flaky stuff on top

So this is the second time this has happened, im getting this sort of soft flaky yeast looking floaties that sit on top when fermenting, i just use the extract kits, usually coopers pale ale but this one i did the dark ale everything like normal no changes but i get this flaky stuff on top.
When this happened on my batch before this one i bottled it and after 2 weeks it was even carbonated and most of the bottles have that flaky stuff on the top of it :-( can someone please explain what it is and how it came about so i can stop it

Have you got a pic? Its quite usual to get a lot of weird things on the top of a fermenting wort, but in the bottle is another matter.
