Lid on until boil starts then lid of until the end when I put the lid back on for part of the whirpooling.
I only have a three ring burner with a standard reg.

Lid off and boil the bejesus out of it !
I boil with the lid off, however as I am doing mini mashes (15l batches, I add 3l of water to fermenter), and boiling on SWMBO's stovetop, I leave the lid on til it gets going (30 min) :huh: , then lid off all the way.
I would be interested in hearing what rate of evaporation different people have with the different methods as i am trying to bed down my efficiency a little more and would love to get a cross section of what the broader AHB community is acheiving in this regard.
