I've had two packs of Wyeast 1768 English Bitter in the fridge, MFG 29 December 2013 - that's 15 months.

Smacked them on Thursday afternoon, it's now Sunday Morning.

No special treatment except for a good shake whenever I was passing.

From time to time on the forum we get panicked Wyeast first users whose three month old pack hasn't swollen after a couple of days, this could be a useful picture to direct them to.

I have had similar experience. I believe it is handling and storage conditions. As I have had aged failures as well.
Remember MHBs 5 yo American ale? Smacked and activated over the long weekend,
I believe that a mechanical formula to predict/estimate viability, is just that, an estimate only, it doesn't take individual circumstances or variation into account.
