Could be the dirty ball valves that caused it. I have read of that happening before. Guys going through all this cleaning of everything else first only to find the ball valve on their kettle or whatever is full of shit. Clean that out and all of a sudden no infections anymore.
I only have an urn so just the one valve but I dismantle it and soak it every 4-5 batches to prevent buildup. Most times I take it apart it's pretty clean from the boiling water I run through it after every brew day but I still like to soak the innards in proper cleaner just to be sure.
 
And go back to the cheap-ass bottling wand - easier to clean/sanitize and you can replace it every so often at little cost.
